Social Work and Case Management

As a patient at UT Southwestern Medical Center, you’re in good hands in every aspect of care, medical and otherwise. Case Managers are assigned to each inpatient-care area. They’re trained to help patients and family members address financial, social, and emotional problems that relate to illness or hospitalization.

Case Managers also help patients and families cope with long-term illnesses, rehabilitation, and discharge planning. They can assist with arrangements for the following services:

Discharge from the Hospital

Discharge times may vary from patient to patient. To ensure the best transition possible, we begin preparing patients for discharge the day before they’re sent home.

We ask that patients and families consider the following issues prior to discharge:

  • Address all questions and concerns with your physician or nurse before leaving the hospital.
  • Be aware of the date and time the patient is to be discharged.
  • Be certain there is someone to assist the patient at home, if needed.
  • Find out whether supplies and/or equipment are needed at home.
  • Make arrangements for transportation prior to the day of discharge.
  • Make sure the patient (or a responsible party) can pay for prescriptions.
