For Your First Appointment
Please bring all pertinent medical records to your first appointment, or verify that our staff has received them from your referring physician. These records can include reports, X-ray films, and test results.
New patients should also bring the name of their insurance provider and policy/group number, as well as the name and telephone number of their referring physician.
We also recommend that you bring a pen and notepad to take notes during your first appointment. A family member or a friend is welcome to accompany you.
