Preparing for Your Appointment

At UT Southwestern’s Clinical Center for Sleep and Breathing Disorders, your clinical evaluation starts with a comprehensive questionnaire that you complete and bring to the first appointment. You will then meet with one of our sleep medicine clinical providers to formulate a diagnostic and treatment plan based on your individual needs and goals for therapy.

New patients should bring the following items to their first appointment:

  • All pertinent medical records (or verify that our staff has received them from your referring physician)
    • Reports
    • Test results
    • X-ray films
  • Insurance provider information, including and policy/group number
  • Name and telephone number of the referring physician
  • List of current medications and/or supplements

CPAP/BiPAP Support Group

UT Southwestern offers a CPAP/BiPAP Support Group where patients can meet others using these treatments and learn more about sleep and breathing disorders.

Meetings include an interactive format led by sleep technologists who review common questions regarding CPAP therapy. Topics include how to clean masks, tubing, and equipment, plus weight reduction strategies.
